Accept Applications from the Public in Person
Explanation:
1. People applying in person for cabinet s, towers withdrawal, excavations, tomb foundations, changes, etc., must prepare relevant documents (Under certain circumstances, original documents can be supplemented later to complete the application).
2.The case officer will review the documents and review key points are:
(1) Name of the applicant's household registration transcript and the deceased's household registration cancellation transcript, kinship, and where the domicile place is registered, year of residence (must have lived in Taoyuan City for one year, to be as a citizen), ID card number, note text (supporting evidence: date of birth and death, residence information, relatives, kinship, etc.), the deadline of household registration transcript (within three months of application).
(2) If the content of the identity document is not one self’s, the applicant must have the ID card and seals of the client, the ID card and seal of trustee, and fill in the power of attorney for the application.
(3) Certificate of excavation of the deceased, certificate of removal, certificate of tower withdrawal, cremation permit, official documents of household administration (evidence of no household registration), relevant official documents (certificate of relocation due to public works: notification letter, certificate of receipt, inspection and evaluation inventory, certificate of compensation, etc.). The relevant information of the deceased and the date of document issuance (within three months) shall be carefully checked.
(4) Regarding the application materials, in addition to the relevant documents, there could be an affidavit completed to prove the truth of the application.
(5) Fill in the kinship form to show the relationship between applicant and the deceased, or explain the kinship, and prove it.
(6) Pay attention to the official documents and laws passed down from the superiors (city halls, funeral houses, and other public offices); pay attention to relevant preferential or restrictive regulations, to avoid and reduce the application defects.
(7) When accepting the documents submitted by the public, if anything negotiable, you must check the relevant provisions and review them. Besides, you must also respond to the supervisor as soon as possible for investigation and evaluation before proceeding.
(8) After the people complete the application process, the case officer can take the initiative to remind the people to submit the supplement documents (within the deadline, before the time when ashes/skeleton urn is set in the Ossuary Tower, so called “Jinta”), and that they must pay the fees to the Farmer’s Association of Dayuan District (within three to five days, and return the fifth section of the form). Also inform the correct date of Jinta (supplement notice or modification to the date and time), and that the excavation photos (prior, in process, afterwards) shall be completed.
